Hospice RN Case Manager

This RN position offers a $10,000 sign-on bonus and a supportive, nurse-led environment focused on meaningful patient care.

Responsibilities include assessing, planning, and coordinating hospice care, leading an interdisciplinary team, and providing emotional support during end-of-life stages.

Qualifications: RN license, at least 1 year clinical experience, strong assessment skills, reliable transportation, and ability to lift 35 lbs.

Preferred: BSN, hospice or home health experience, familiarity with HCHB.
