The PHP Clinician conducts patient evaluations, intake interviews, assesses issues, and formulates diagnoses to recommend appropriate treatment.
Assist in developing and modifying treatment and continuing care plans, while providing case management and discharge summaries.
Manage crises, coordinate with external providers, and participate in ongoing education to maintain expertise and meet credentialing standards.
Attend meetings, supervise, and ensure compliance with payer and regulatory requirements through accurate documentation.
Serve as a resource in the community, present educational sessions, and mentor interns and students.
Requirements include a Master’s in counseling or related field, 1-2 years supervised experience, familiarity with technology, and preferred licensure (LICSW, LMHC, or LMFT).
