Physical Therapist / PT

The On-Call Registered Nurse provides hospice care by responding to after-hours calls, assessing patients, and adjusting care plans as needed.

Responsibilities include conducting comprehensive assessments, administering medications, and educating patients and families on self-care.

Supports patients through crisis management, documentation, and communication with physicians and community resources.

Collaborates with interdisciplinary teams, supervises aides, and ensures a safe, compassionate environment.

Requires RN licensure, 1-2 years of recent acute and home care experience, CPR certification, and a valid driver’s license.
