Job Description

Yoh is seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive primary and occupational healthcare services at a leading on-site clinic in Modesto, CA. This is a full-time contract opportunity with no weekend requirements.

Position Overview:

The provider will deliver high-quality, patient-centered care to individuals age 2 and older. Responsibilities include a full spectrum of primary care, women's health, acute illness management, and occupational health services. This includes both scheduled and walk-in appointments, conducted in-person, via video, or over the phone.

Schedule: Monday–Friday 8:00 AM – 5:00 PM No weekends

Contract Duration: May 12, 2025 – September 30, 2025 potential for extension

Key Responsibilities:

See approximately 15 patients per day across a variety of visit types

Conduct primary care evaluations (acute, chronic, well visits, pediatric, sports physicals)

Perform occupational health assessments (including work comp injuries and follow-ups)

Manage urgent cases (sprains, strains, minor lacerations, fractures, and chemical exposures)

Conduct CLIA/POCT testing and specimen collection (e.g., COVID, Influenza, Strep A, UDS)

Coordinate imaging and emergency referrals when necessary

Complete documentation within 24 hours in EPIC and OHM EMRs

Respond to refill requests, lab results, and messages within 48 hours

Ensure all occupational paperwork is completed by the end of each workday

Clinic Resources:

On-site lab with daily pickups by Quest Diagnostics

CLIA-waived testing: FSBG, urinalysis, rapid drug screens, Strep A, Flu A&B, COVID-19, Hemocue

Breathalyzer and drug screening for work comp evaluations

Contracted imaging center and emergency referral protocols.Requirements:

Active NP or PA license in California

Minimum of 2 years of clinical experience (Primary care or occupational health preferred)

Proficiency in EPIC and OHM EMR systems

Strong clinical judgment, autonomy, and organizational skills
