Job Title: Case Manager / Home Health Aide (Joint Role – Behavioral Health & Home Health)
Location: Columbus, OH (Travel Required, including to surrounding counties)

Reports To: Assistant Director (CBHA) / Chief Executive Officer (Moonlight) About the Role
We are seeking a skilled and motivated Case Manager / Home Health Aide to support clients across two partner organizations: Moonlight Home Health and Comprehensive Behavioral Health Associates (CBHA). This unique, dual-role position is not entry-level and is designed for an individual with experience in care coordination and direct client support.
The goal of this position is to help identify and enroll Moonlight clients into behavioral health services with CBHA, and vice versa — ensuring that individuals receive holistic care addressing both physical and mental health needs. Key Responsibilities Case Management – CBHA
Conduct client intakes and assess eligibility for behavioral health services
Coordinate appointments, referrals, transportation, and follow-up care
Monitor client progress and maintain up-to-date documentation in EMR systems
Collaborate with the behavioral health team to support treatment planning and service delivery
Build rapport with clients and promote engagement in both behavioral and home health programs Home Health Aide – Moonlight
Assist clients in their homes with personal care: bathing, dressing, toileting, grooming, mobility, and safety support
Perform light housekeeping and meal preparation tasks
Observe and report changes in clients’ physical or emotional conditions
Provide companionship and ensure a clean, safe, and respectful home environment Qualifications
High school diploma or equivalent required
Minimum 1–2 years of experience in case management, behavioral health, or home health support
Certification preferred: HHA, PCA, or CNA
Valid driver’s license and reliable transportation required (daily travel is expected)
Strong interpersonal, organizational, and documentation skills
Able to pass background checks, screenings, and complete required training
Comfortable using EMR platforms and basic computer applications Ideal Candidate
Experienced in working with adults with physical, mental, or developmental health needs
Confident in both care coordination and hands-on personal care
Organized and detail-oriented with the ability to work independently
Committed to helping clients navigate services and improve their overall well-being
Comfortable traveling throughout the Columbus region and surrounding counties, including Mahoning County when needed Compensation & Benefits
Hourly Pay: $14/hour
Mileage reimbursement for travel outside standard area
