Now hiring a full-time Nurse Practitioner to come join a wonderful outpatient geriatric clinic in North Charleston, SC

JOB SUMMARY: The PCP Nurse Practitioner has the responsibility for providing direct and indirect care to the participants in the PACE Health Clinic and Day Center. Works under the direct supervision of the Medical Director/PCP. Manages complex clinical situations for which he/she is responsible.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Performs health histories, physical exams, and participant teaching. Orders, interprets, and follows up on lab tests and results. Performs selected procedures and documents care in writing in EMR. Initiates emergency measures as indicated. Recognizes abnormalities that justify immediate referral for intervention by others.

Initiates, evaluates, reviews and changes orders for selected medications and diagnostic studies/treatments within attending physician approved protocols.

Acts as liaison with physicians, nurses and other healthcare professionals providing education, communication and support for healthcare staff, participants, and families. Directs participant management in conjunction with physician staff.

Participates as a member of the Interdisciplinary Team.

Attends staff meetings and takes part in participant care planning..

Evaluates and treats participants in their homes, assisted living facilities, nursing homes, and hospitals when medically indicated.

Schedule: M-F 8am – 4:30

Patients per day: Typically, the expectation is 15-20 patients, ensuring they are never rushed. (Subject to change based on site needs)

QUALIFICATIONS:

• Master’s degree in clinical nursing from accredited Nurse practitioner program.

3-5 years nursing experience with Nurse Practitioner experience preferred.

• NP licensure in perspective state and a Family certification from Nurse Practitioner program.

Pay: $120k-$140k
