Flexibility is needed for community events after work hours.\n This position will be in the field 50-75% requiring local travel in Fort Worth.
This position will travel four to six times a year to Amarillo and Lubbock, Texas.

Assess patient needs and help plan, implement, and evaluate healthcare with the medical team.
Work autonomously and with the team.
Also serve as advocate, teacher, and coordinator for patients.\n Essential Functions of the Role\n\nCare for patients in all aspects by evaluating their health based on doctor's orders and creating a healthcare plan.\nSchedule outpatient tests for diagnosis, stay in touch with the healthcare team.\nVisit doctors often, support patient-office communication.\nCollaborate with healthcare professionals for optimal patient care.\nConduct follow-ups every 3 months and 1 year.\nMaintain up-to-the-minute and comprehensive nursing records, also updating patient details in the Patient Navigator database.\nAttend case conferences to expand your knowledge.
Participate in community service events.\nWork together on improvement projects to enhance service for patients, families, and the healthcare system.\nWelcome patients and families, offer facility details, directions, and general information.\nAssist in guiding patients and families through the hospital, especially to hard-to-find places.\n\n Key Success Factors\n\nExceptional abilities in guidance, decision-making, and delegating tasks.\nUnderstanding of care coordination, resource allocation, and usage management.\nProficiency in managing patient care and conducting patient assessments.\nExcellent communication skills.

QualificationsA Bachelor's degree.At least 3 years of relevant experience to the table.Registered Licensed Nurse.You have, or are willing to obtain within your first 30 days, a Basic Life Support (BLS)
