Job Description
Duties and Responsibilities:
Encourage individuals in personal growth through the promotion of autonomy and aid in
daily activities
Responsible for creating and aiding individuals in daily responsibilities including meal
preparation, cleaning, personal hygiene, and other requirements.
Coordinate and aid in athleisure activities within the office or community.
Ensure your Individuals Plan of Service is being followed daily including but not limited
to their medical, dietary, psychological, and physical needs.
Administer medication/treatments as recorded in their doctors orders and company
policies according to their Level of Need.
Receive direct supervision from Senior Director of Day Services.
Record and maintain the following records in a timely, consistent and accurate manner:
program and behavioral data, medical and medication records, including prescriptions,
financial receipts, appointment books (including all scheduling as required); administer
evaluations and assessments to individuals; transition book or other applicable staff
communication books; all other related records as required. Complete Therap as
required.
Maintain consistent communications between supervisors, programs, and
parent/guardians as needed.
Maintain necessary qualifications and certificates consistent with DDS and Safeway
program standards including PMT, CPR, Mandated Reporter and Abuse and Neglect
training.
Remain well-informed of and advocate for your individuals rights and well-being while
maintaining confidentiality according to agency policy.
Assist in or lead emergency/behavioral situations as necessary, using ones PMT and
CPR training.
Perform necessary errands and responsibilities to aid in the maintenance of the program
success as necessary.
Maintain a flexible schedule and along with an openness to provide relief coverage
within office, residential, or community settings.
Qualifications:
One to two years of experience working with persons with intellectual disabilities
preferred
Valid Drivers License required and personal vehicle preferred.
Must maintain a sufficient driving record.
Must drive yourself and your individual among office, resident, and community sites
throughout various neighborhoods and among various weather conditions.
Must be able to work at various locations depending on the work assignment given.
Must have/be willing to attain PMT, CPR, Mandated Reporter, and Abuse and Neglect
training. Must maintain these certificates at all times.
Understanding of Microsoft Office and the internet.
Physical Requirements:
Provide hands on personal care which may include toileting, feeding, cleaning and all
other activities of daily living skills.
Ability to transfer individuals as necessary.
Have physical endurance to use physical crisis intervention techniques which may
include restraining individuals.
Provide physical intervention as necessary which may include lifting, mobility, first aid,
and safety evacuations.
Ability to lift up to 50lbs.
Have the physical endurance to use PMT holds.
Have the physical endurance to be able to lift and transfer individuals as assigned.
