The Certified Coder position will be part of our Population Health group and work collaboratively with our CEO, Coding and Compliance and Population Health Navigators on all areas of risk adjustment coding to ensure coding efficiency and optimization of funds related to chronic condition coding and reimbursement.
Hybrid Certified Coder Family Practice Responsibilities:
Assign International Classification of Disease system-10(ICD) CM codes according to AHA – AMA Guidelines, CMS and NGS.
Review patient charts to identify gaps in documentation and report findings to the management team for planning of education on processes to correct documentation gaps and close gaps in care.
Work with the management team to develop processes for problem list tracking, problem list update, and identify efficiencies and solutions for long-term revenue optimization.
Work to meet specific goals as developed by the management team and the Population Health department on coding projects and HCC initiatives.
Ensure compliance with all applicable Federal, State and/or County laws and regulations related to coding and documentation guidelines for Risk Adjustment.
Quickly identify incorrect coding and compliance trends; analyze, investigate and communicate suspected problems with resolutions.
Participate in quality improvement initiatives.
Hybrid Certified Coder Family Practice Education/Experience:
One of the following certifications is required: CPC /CPMA/ CRC/ CCS-P/ CCS/ RHIA or RHIT
2 years minimum of inpatient or outpatient experience in ICD-10-CM code assignment
Previous experience using electronic medical record (EMR) system required ; Medent EMR experience preferred
Strong knowledge of ICD-10-CM coding
CRC candidates with experience preferred
