Nurse Case Manager II Compensation
The pay for this position is $55.64 per hour
Benefits are available to full-time employees after 90 days of employment
A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Nurse Case Manager II Highlights
This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
The required availability for this position is 8am–5pm EST
Class start date is 6/2; if not cleared to start on 6/2, the next available start is 7/7
This is a fully remote position; candidates must reside in an EST time zone and work EST hours
Nurse Case Manager II Responsibilities
Develop proactive plans of care to enhance outcomes and member wellness
Use clinical tools and data to evaluate members' needs and benefits
Apply clinical judgment to reduce risk factors and address complex health and social issues
Conduct assessments using multiple data sources to address co-morbid and functional diagnoses
Assess need for clinical resources or interdisciplinary referrals using a holistic approach
Collaborate with supervisors and key healthcare stakeholders to overcome care barriers
Present member cases at interdisciplinary case conferences
Use case management processes per regulatory and company policies
Use motivational interviewing to maximize member engagement
Nurse Case Manager II Requirements
Registered Nurse with active license in good standing
Must have a Pennsylvania RN license
If residing outside PA, candidate must have a compact license
High School Diploma or GED required
Minimum 3-5 years of clinical practice experience
Minimum 2-3 years in case management, discharge planning, or home health care coordination
Strong computer proficiency including Microsoft Word, Excel, Outlook, and PowerPoint
Ability to work independently and manage multiple systems remotely
Attendance is mandatory for the first 90 days
Preferred Qualifications
Bilingual candidates strongly desired
Certified Case Manager (CCM) preferred
Additional state licenses preferred
