RN Nurse Case Manager

Location: Eastern Time Zone

As a Registered Nurse (RN) Case Manager, I was responsible for the coordination, implementation, and evaluation of comprehensive care plans for patients across diverse clinical settings. I utilized clinical knowledge and critical thinking skills to ensure quality outcomes, cost-effective care, and optimal patient satisfaction.

Key Responsibilities:

Assessed patient health status, developed individualized care plans, and facilitated timely interventions in accordance with clinical guidelines and best practices.

Collaborated with interdisciplinary teams including physicians, social workers, therapists, and insurance providers to coordinate services and streamline transitions of care.

Conducted thorough chart reviews, documented case notes, and maintained accurate and up-to-date records within electronic medical records (EMR) systems such as Epic, Cerner, or other platforms.

Monitored patient progress, evaluated the effectiveness of care plans, and adjusted interventions based on changing needs and health conditions.

Served as an advocate for patients and families, ensuring access to necessary healthcare resources, support services, and community programs.

Educated patients and caregivers about disease management, medications, treatment plans, and discharge instructions to promote health literacy and prevent readmissions.

Managed a diverse caseload, prioritized urgent needs, and met performance and compliance metrics related to utilization review, cost containment, and quality improvement.

Conducted telephonic and/or in-person outreach to ensure continuity of care and to close gaps in care.

Complied with HIPAA regulations and maintained confidentiality of all protected health information (PHI).
