Location: Spokane WA (HYBRID – Home, Office & Local travel throughout Spokane County)

Job Schedule: Full Time, M-F 8:00AM-4:30PM (+Participation in On Call Rotation)

About the Role

The Professional Care Manager & Counselor works autonomously, using discretion and independent judgement to provide clients, families, or vulnerable populations with medical and psychosocial support needed to cope with chronic, acute, or terminal illnesses. Includes administering client assessments, care planning, advising families care givers, providing patient education and counseling and making necessary referrals for other social services. Counselor specific duties to include individual or family counseling.

The ideal candidate for this role is for this role is a licensed clinician with strong care coordination skills, deep empathy for aging and medically vulnerable populations, and a proven ability to work independently across home, community, and clinical settings.

Job Duties

Care Management:

Assess Clients in their homes; biological, psychological, and sociological state; identifying barriers to client progress through interviews and a review of patient records

Create Personalized Care Plans with SMART goals (specific, measurable, achievable, relevant & time-bound)

Connect clients and/or families with referrals and community resources to assist in recovery from mental or physical illness and provide access to services such as financial assistance, legal aid, housing, job placement or education

Coordinate Care in home, rehabilitation, or senior living community utilizing clinical data and behavioral health experience to ensure service efficacy

Plan Discharges: from care facilities to home or other care facilities, ensuring safety

Revise Care Plans as needed to align with changes in client status, while also diligently tracking, assessing, and documenting client progress in accordance with the measurable goals outlined in the treatment and care plan.

Advocate for clients to help resolve crises & support them in their personal health journey

Document, chart, and record client information, billable hours, and client notes.

Meet Billable expectations and billing requirements.

Transport Clients to and from their doctors' appointments, community resource visits, and more.

Participate in On Call rotation with other Care Managers as assigned.

Licensed Counseling:

Diagnose mental health disorders according to DSM-5 criteria

Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life

Use Evidence-Based Modalities (eg CBT, DBT, trauma-informed care)

Lead Support Groups and counsel family members to assist them in understanding and/or processing life transitions.

Document, chart, and record client information, according to HIPAA requirements.

Qualifications

LICSW, LICSW(A), MSW, LMHC, LPC; or related degree

Active license to provide counseling in Washington state

Certified Care Manager (CCM or CMC) preferred

3+ years experience in direct clinical treatment required

2+ years experience in community-based Healthcare (home health, public health, home care) preferred

Current driver’s license and auto insurance and the ability to drive to and from appointments.
