LMHC/LCSW-Wauchula and Winterhaven

Job Description JOB SUMMARY The duties of a LMHC or LCSW include, but are not limited to, commute to multiple post-acute care facilities every day, making rounds, communicating to patients, facility staff members, providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients.

Work with Psych division providers within the post-acute facilities.

MUST HAVE A FLORIDA LICENSE Monday-Friday 15-20 Patients per day/On Site 1099/W2 Flexible hours PRIMARY FUNCTIONSExamine patients and record historyAdhere to proper charting protocolsDiscuss with Physician and other health care professionals to prepare comprehensive planSubmit health care plan and goals of individual patients for periodic review by PhysicianPromote health by advising patient on diet, hygiene and prevention of diseaseRefer to specialist as required for patient's well-beingReport deaths and contagious diseases to governmental authoritiesOther responsibilities as assigned EDUCATION AND EXPERIENCEMust have and maintain current State of Florida license in good standingMust be LMHC/LSW in the State of FloridaCurrent National Board Certification KNOWLEDGE, SKILLS, AND ABILITIES Effective written, verbal and electronic communication skillsShow initiative, creativity and problem-solving abilityAbility to collaborate with colleagues to provide quality healthcare ADDITIONAL QUALIFICATIONSPrior experience using an electronic medical records systemBilingual a plus, but not required (English/Spanish) RELATIONSHIP REPORTING Reports to the Supervisory Psychiatric Physician / Executive Director PHYSICAL REQUIREMENTSAbility to communicate effectively with patientsAbility to collaborate with colleagues as necessary to provide quality health careAbility to use a computer, printer and scannerAbility to sit, stand or view a computer screen for extended periods of timeAbility to perform repetitive hand and wrist motions for extended periods of timeAbility to use hands and fingers to handle or feelAbility to reach with hands and arms PI542edb81fef4-25405-37704142
