JMuir Walnut Creek CASE MANAGER. ** MUST HAVE RECENT ACUTE CARE HOSPITAL CASE MGMT EXP ** • Orientation dates: 6/3 or 6/17 • Shift: 8 hour days (0800-1630) - 40 hours per week *Will float between Concord and Walnut Creek campuses as needed • Duration: 13 weeks •REQUIRED : Active CA license • Must have 2+ years experience in an acute care hospital •PREFERRED : previous travel and EPIC exp

Required Qualifications

Floating required to sister campus, John Muir Concord if needed

Licenses: RN-CA(Copy needed)

Certifications: BLS(Copy Needed)

Work History: Should not be more than 30 days of gap in entire work history

References: 1 Reference in the last 12 months
