Vitality NP Services is seeking a Nurse Practitioner to become a part of our team!

Full or part time position open for NP in a skilled nursing facility in Elizabeth, NJ.

Offering flexible hours and days. No evenings, holidays, or on call required!

*Facility would like 2 weekdays and 1 weekend but schedule can be negotiated*

Responsibilities:

Assessment of all recent admissions and acute issues of both subacute and long-term care patients

Prevention of needless hospital transfers through early assessment and interventions

Communication and collaboration with primary care and specialists

Qualifications:

Previous RN experience - NEW NP GRADS WELCOMED!

Ability to work as an interdisciplinary team member

Good critical thinking/ assessment skills and the ability to function in a fast-paced environment

Excellent written/verbal communication skills and computers
