In this role, you will have the unique opportunity to establish and nurture relationships with residents while overseeing their long-term and preventive care needs.

You will work collaboratively with Curana Health’s care teams and facility staff to deliver exceptional, consistent care in the convenience of the patients' residences.\n\nEssential Duties & Responsibilities:\nPatient Panel Development: Build and maintain a dedicated patient panel by establishing relationships with new residents and managing ongoing care for established patients within assigned facilities.Direct Patient Care: Conduct comprehensive assessments, manage chronic conditions, and provide preventive care services for residents, ensuring continuity of care as their primary physician.Routine Rounds: Perform regular rounds at assigned facilities, addressing both chronic and acute health concerns.Care Coordination: Collaborate with facility staff, family members, and interdisciplinary teams to create and manage personalized care plans tailored to each resident's needs.Documentation and Compliance: Ensure accurate, thorough documentation in the EMR system, adhering to regulatory requirements and Curana Health’s policies.Patient Education: Educate residents and their families on health management, preventive care, and available resources.Quality Assurance: Participate in quality improvement initiatives, analyzing performance metrics to continually enhance patient outcomes.
