Field Medical Case Manager

Job Description

A-Line Staffing is now hiring Field Case Manager in Pinellas County, FL. The Field Case Manager would be working for a Major Health Plan and has career growth potential.

Field Case Manager Highlights:

• Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST

• Hybrid role: Remote for training and at least 1 day per week; fieldwork required in Pinellas County, FL

• Pay Range: $22–$25 Hourly + Mileage Reimbursement

Field Case Manager Responsibilities:

• Manage a caseload of health care members with long-term care needs

• Conduct member assessments and document care plans

• Perform monthly and quarterly member contact (80% travel required)

• Complete in-person assessments with members, caregivers, or providers

• Monitor service delivery and conduct follow-ups via in-person or phone

• Coordinate referrals and ensure timely delivery of provider services

• Identify and address care plan deficiencies

• Collaborate with discharge planners and other health care professionals

• Assist members with complaints and appeals

Field Case Manager Qualifications:

• Bachelor’s degree in a relevant field

• 2+ years of medical case management experience

• Proficiency in Microsoft Office and electronic medical records

• Valid driver's license and reliable transportation

• High-speed internet and a quiet, secure home office

Field Case Manager Preferred Experience:

• Fieldwork/home visit experience

• Background in home health, discharge planning, or long-term care

• Bilingual in Spanish and English is a plus
