RN Field Case Manager Details and Compensation:

Location: Fond du Lac and Winnebago Counties – 100% onsite

Payrate: $53.93/hr

Required Availability: Mon-Fri 8a-430p no on call

RN Field Case Manager Summary and Highlights:

The RN Field Case Manager role involves performing five patient visits per day and working closely with healthcare teams to support high-risk members in their homes. This is a 100% onsite field position - 5 visits per day.

This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Field Case Manager Responsibilities:

Conduct comprehensive assessments of high-risk patients.

Collaborate with primary care providers to develop and execute individualized care plans.

Identify and monitor cost-effective, appropriate levels of care.

Coordinate support services such as housing and transportation to enhance treatment compliance.

Manage care transitions in collaboration with social workers, discharge planners, and other care team members.

Document clinical, functional, and fiscal outcomes in detail.

Identify health promotion and illness prevention opportunities.

Serve as a patient advocate, ensuring privacy and confidentiality.

RN Field Case Manager Requirements:

Active RN license in the state of Wisconsin.

Associate’s or Bachelor’s Degree in Nursing or related field.

Current CPR/BLS certification.

Wound care experience required.

Prior experience with Electronic Medical Records (EMR) required

HCHB is highly desired, MatrixCare or similar platforms are acceptable.

Case Management Certification is a plus.

Reliable vehicle, valid driver’s license, and current auto insurance (mileage reimbursed at $0.53/mile)..

Attendance: Commitment to mandatory attendance during the first 90 days.
