Position: Hospice RN Case Manager
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (some on-call required, including evenings, overnight, 24-hour shifts, or covering weekends)
Location: Fond du lac, WI
Position Overview:
We are seeking a compassionate and dedicated Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a critical role in delivering high-quality care to our patients by performing comprehensive assessments, creating personalized care plans, and ensuring continuous, cost-efficient care delivery. You will work closely with primary care providers, specialty resources, and other healthcare professionals to improve patient outcomes and promote health and wellness.
Primary Responsibilities:
Perform comprehensive assessments of high-risk members to determine appropriate levels of care.
Collaborate with primary care providers to implement individualized, comprehensive care plans with specific interventions.
Ensure timely and cost-efficient care by identifying and reassessing care needs as required.
Establish non-medical support systems, such as housing and transportation, to ensure patient compliance with treatment plans.
Coordinate care transitions as necessary, working with physicians, social workers, discharge planners, and claims professionals.
Engage specialty resources as needed to optimize care resolution.
Maintain detailed records of clinical, functional, and fiscal outcomes.
Identify opportunities for health promotion and illness prevention.
Intervene quickly to prevent adverse patient occurrences and minimize poor outcomes.
Act as a patient advocate by ensuring privacy and confidentiality at all times.
Qualifications:
Associates or Bachelor’s Degree in Nursing or a related field.
Current, unrestricted state Registered Nurse license.
Case Management certification.
Ability to analyze complex medical information and make informed, rational decisions.
Strong technical skills; familiarity with HCHB (Home Care Home Base) is highly preferred, but candidates must be tech-savvy if unfamiliar with this platform.
