Coordinates and manages patient care and disposition planning for the Inpatient Psychiatric Unit.
Job Description:
Essential Functions:
Coordinates inpatient and post-discharge care, scheduling, and reporting.
Provides disposition planning and maintains disposition data.
Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
Coordinates available NCH and community resources to enhance client care.
Provides education and support to patients and their families regarding treatment options and resources.
Completes documentation of discharge and care coordination activities in Epic and other related administrative paperwork in accordance with policy standards.
Education Requirement:
Bachelor's Degree in social work, counseling, psychology, or a related field, required.
Licensure Requirement:
LSW, required,
Skills:
Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives.
Ability to work with the hospital’s electronic medical records system.
Ability to work and problem-solve with minimal supervision.
Ability to adapt to change positively in workflow and process.
Spreadsheet, data entry and tracking, and computerized scheduling skills.
Experience:
Experience providing and managing behavioral healthcare to children, adolescent and families, required.
