Assesses needs/goals to maintain or enhance the psychosocial, mental and emotional needs of our residents.
Identifying residents' psychosocial, mental and emotional needs along with providing, and/or aiding in the access of services to meet those needs.
When appropriate, work with resident, family and other team members to plan discharge. Arrange home care services; assist with transfer to another location.
Act as a liaison between resident, family and interdisciplinary team.
Document all resident care plan information and follow State and Federal codes.
Advocate for residents and participate in solutions to identified problems.
Collaborate with other departments to evaluate residents' psychosocial wellbeing, mood, and behavior concerns.
When necessary, refer residents and/or families to community resources to assist with concerns in areas of finance, housing and additional services.
Take leadership role in investigation of resident and/or family concerns, follow through to conflict resolution and reporting to state and other authorities as necessary.
Perform assessment of residents at admission, upon change in condition and/or annually to identify and seek ways to support residents' individual needs.
Create, review, and update plan of care to support residents' individual needs.
Promoting actions by staff that maintain or enhance each resident's dignity in full recognition of each resident's individuality.
Bachelor's Degree in Social Work or other related behavioral science. Social Work certificate for state of Wisconsin, preferred
Three to five years of experience in one of the following environments a Long Term Care, Assisted Living or Community Base Facilities
Ability to document resident care plans concisely and clearly. Ability to be patient, sensitive and receptive to both the residents and their family's needs.
Must be available for on-Call for weekend concerns or resident admission on a rotation basis.
Attend in-services/classes concerning social workers field and to maintain social work certification.
