Summary Description: Community Oriented Recovery and Empowerment (CORE) provides opportunities for Medicaid beneficiaries with behavioral health conditions and substance use disorders (SUD) to receive services in their own home or community. CORE utilizes a recovery model of care, which emphasizes and supports a person's potential for recovery by optimizing the quality of life and reducing symptoms of behavioral health conditions and substance use disorders through empowerment, choice, treatment, educational, employment, housing, and health and well-being goals. Recovery is generally seen in this approach as a personal journey rather than a set outcome, and one that may involve developing hope, a secure base and sense of self, supportive relationships, self-direction, social, and coping skills. The Clinician is responsible for the provision of direct services to individuals with persistent psychiatric disabilities. The Clinician will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible, he/she will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.
Key Responsibilities:
The Clinician will provide services to help individuals with behavioral health diagnosis to achieve stability and functional improvement in the following areas: daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community integration.
The Clinician will provide mobile treatment and rehabilitation services to individuals who have difficulty engaging in site- based programs who can benefit from off-site rehabilitation or who have not been previously engaged in services, including those who had only partially benefited from traditional treatment or might benefit from more active involvement of their family of choice in their treatment.
Weekly schedule may include evenings and weekends.

Requirements:
The Clinician must hold one of the following licenses or permits from the New York State Education Department: Certified Rehabilitation Counselor (CRC), Creative Arts Therapist, Practical Nurse, Psychoanalyst, Psychologist, Marriage and Family Therapist, Mental Health Counselor, Nurse Practitioner in Psychiatry, Physician Assistant, Registered Nurse or Social Worker.
The Clinician must have the training and experience to provide direct services related to the treatment of mental illness and substance use disorders.
Basic Computer knowledge is required.
Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
The Clinician must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
If driving agency vehicles, the Clinician must maintain current New York State Defensive Driving certification within 30 days of hire.
Must maintain pre-employment clearances by applicable regulatory clearances as required.t.
The Clinician must be legally eligible to work in the United States.

Salary Range: $55,000- $64,400, based on departmental budget, qualifications, experience, education, and licenses. This range represents the agency’s good faith and reasonable estimate of the range of possible compensation at the time of posting.
