We are currently seeking a dedicated and reliable Home Health Registered Nurse (RN) for a 13-week contract in Portage, Wisconsin. This is a night shift position, working 8-hour shifts providing one-on-one skilled nursing care in the comfort of the patient's home. The ideal candidate will have a strong clinical background and the ability to work independently. Key Responsibilities:

Deliver compassionate, evidence-based nursing care in a home health setting

Perform physical assessments and document patient progress during night shifts

Administer medications, treatments, and monitor vital signs as ordered

Communicate and coordinate with the patient's care team, including physicians and family members

Ensure documentation is accurate and completed in a timely manner

Respond to changes in patient condition promptly and appropriately

Maintain a safe, clean, and supportive environment in the patient's home Requirements:

Active RN license in Wisconsin (or compact license)

Minimum 2 year of RN experience (Home Health experience preferred)

Night shift availability for the full 13-week assignment

Valid driver's license and reliable transportation

Current BLS/CPR certification

Strong time management and documentation skills

Ability to work independently in a home care environment Contract Details:
