JOB SUMMARY:

Provides services and support to individuals in the community as part of a multi-disciplinary care team to meet people where they are and help with their immediate social emotional needs.

Respond to individuals in the community and facilitate de-escalation and safety planning.

Serves as a point of contact for referring community partners and healthcare team to help coordinate referrals to treatment and help individuals reduce barriers and overcome obstacles.

Direct patients to sources of emotional, financial, administrative, or cultural support

Build working relationships, solve problems, and support patients as they learn to self- navigate and address needs related to social determinants of health.

Serve as the clinical lead for the mobile response team. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Provides intervention and support services to patients in the community in collaboration with the multidisciplinary treatment team.

Serves as the clinical lead as deemed appropriate to address identified needs of patients and community.

Serves as a liaison and support for community stakeholders including Local Law Enforcement, EMS, Fire, and non-profit organizations.

Conducts crisis mental health and substance use screenings for patients within scope of practice and provides recommendations for care.

Evaluates psychosocial needs and creates a plan of care for patient and/or family.

Conducts screening for suicide and homicide risk including screening for acute care services and providing recommendations for care and safety planning.

Coordinate services for individuals in need, including but not limited to substance use disorder treatment, counseling, transportation, financial assistance, nutrition, and housing needs.

Stay informed with changes in the service delivery system and seek current information about available resources.

Provide strengths-based, unbiased crisis support to individuals both in person and via phone.

Manage and maintain a client data tracking system, including updated information, demographics and referrals made and followed up on.

Building client relationships and trust by being an effective communicator and listener, while understanding your role’s boundaries and maintaining a professional relationship.

Responsible for networking and outreach efforts to establish and maintain positive working relationships with key community partners.

Perform clinical work with minimal supervision. May be required to occasionally work irregular hours, attend other job-related meetings and/or training, and perform other duties as assigned. Requirements

Managing multiple priorities. EDUCATION AND EXPERIENCE REQUIREMENTS:

Master’s degree in social work or counseling related field required with a minimal of temporary license.

Active plan to earn full licensure (LCSW, LMHC, LMFT).

Trained or willing to become trained in QPR, experience in providing risk assessments, crisis intervention, and experience with substance use disorders.
