We are currently seeking a Part-Time Medical Social Worker (MSW) to join our patient-centered hospice team. If you have a heart for helping others navigate the complexities of end-of-life care and are looking to make a lasting impact in your community, we would love to hear from you.
Position Summary
As a Hospice Medical Social Worker, you will provide critical psychosocial support to patients and families, helping them cope with illness, grief, and end-of-life decision-making. You will serve as a vital bridge between the hospice team, the patient, and the communityensuring that emotional, financial, and resource needs are met with compassion and expertise.
Key Responsibilities
Accept and respond to referrals from the interdisciplinary team for patients requiring social work services
Conduct assessments to evaluate patients psychological, emotional, social, and economic needs
Plan and provide short-term, goal-oriented counseling and support to patients and their loved ones
Connect patients and families to appropriate community resources and support services
Maintain accurate and timely documentation in accordance with the Plan of Care and hospice policies
Participate in interdisciplinary case conferences and provide psychosocial input to care plans
Make scheduled home visits and participate in on-call rotation as needed
Uphold patient rights and dignity, promote comfort, and support end-of-life transitions with empathy and professionalism
What You Bring
Education:
Masters degree in Social Work (MSW) from a school accredited by the Council on Social Work Education (CSWE)
Experience:
Minimum 1 year of social work experience, preferably in a healthcare setting
Licensure & Certification:
Current license to practice as a Social Worker in the state of New Mexico
CPR certification
Valid driver's license with current auto liability insurance
Skills & Qualities:
Strong interpersonal and counseling skills
Excellent communication, documentation, and time management skills
Knowledge of Medicare/Medicaid and community resources
Ability to work independently in home settings
Professionalism, empathy, and emotional resilience
