Case Worker - Foster Care $48K - $55K BOE Monday - Friday 8:00 a.m. - 5:00 p.m. Gahanna, OH

Job Description:

The Case Manager is a degreed social worker, who is assigned direct responsibility for children and foster family caseloads.

the primary duties of the Case Manager sis to ensure foster parents continuously meet the treatment and basic needs of children placed in their care. The Case Manager will report directly to the agency foster care manager or the Director, when the foster care manager is absent.

Job Responsibilities:

Review children's case file to identify trauma history that cause children to enter foster care. Review existing treatment plans to increase understanding on how to develop future treatment plans.

Develop Basic knowledge about the foster families on the caseload.

Supervise assigned caseload through announced and unannounced home and school visits.

Write treatment plans within 30 days of getting placed in the foster home

Research appropriate training tools and information to educate and coach foster parents on crisis management and proper behavioral intervention that promote desired positive change.

Use agency approved forms to collect updates during home visits, which must be entered in the monthly progress report and SACWIS.

Write and submit monthly progress reports to the Director on set dates, and incident reports within 24 hours of occurrence.

Provide foster children regularly scheduled opportunities for agency planned outings and fun events in the community (movies, park, zoo, bowling.

Qualifications & Requirements:

Bachelor degree in Social Work

Already possess a Social Work License, preferred. But those eligible are also welcomed

Prior experience working with children in daycare, school, coaching, or pediatric hospital
