Job Description

Pride Health is currently seeking candidates for a " RN- Hospice Case Manager "_ position that will provide support to our client's healthcare systems. This is a contract assignment _with a high likelihood of extension and a great way to start working with a top-tier healthcare organization!

Location: West Allis WI 53227

Duration: 13 Weeks

Schedule: 40 hours./week

Shift: m-f 08-1630

Pay: $2,900.00 - $3,120.00 per week

Floating Requirements: within service area of Racine and Kenosha counties- with a possibility to float to neighboring areas

On Call, Call Back Requirement: none

Weekend Rotation: none

Holiday Requirements: none

Requirements:

1 year of Home health experience required

Compact or WI license

BLS

Duties

Assess patient needs and develop individualized care plans to ensure optimal health outcomes.

Coordinate with multidisciplinary teams to facilitate patient care across various settings, including emergency rooms and Level II Trauma Centers.

Monitor patient progress and adjust care plans as necessary based on ongoing assessments.

Educate patients and their families about treatment options, medications, and health management strategies.

Advocate for patients within the healthcare system to ensure they receive necessary services.

Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with regulatory standards.

Participate in quality improvement initiatives to enhance patient care processes and outcomes.
