Hospice RN Team Leader

What You Will Do
As a Hospice Team Leader, you will report directly to the Administrator and serves as Administrator in Charge in the absence of the Administrator. Upholds Hospice of Greater Cleveland and McGregor standards of care and assures compliance with federal, state, and local regulations.
Responsible for supervising the implementation of the patient’s plan of care, by receiving referrals, assigning cases, auditing, organizing, overseeing, integrating, and modifying the resources necessary to reach desired outcomes. Confers with Medical Director regarding questions of eligibility.
Collaborates with Administrator and other members of Hospice Team in determining scope and frequency of client services. Collaborates with other health care providers, including those at facilities, about change in condition and client needs.
In collaboration with the Administrator, provides leadership to the department. Leads Interdisciplinary Group (IDG) meetings. Chairs the Department’s Infection Control Committee. Actively participates in the Department’s QAPI program.
Provides oversight and supervision to all members of the IDT. Acts as a resource to clinicians in the field. Participates in the performance review process. Collaborates with Administrator regarding disciplinary actions.
Completes home visits and on-call duties after normal hours, on weekends and on holidays as required.
Serves as a resource to staff in managing patient needs, including collaboration with patients/families regarding goals of care. Provides ongoing education and training to other staff, clients, family members and informal caregivers related to hospice philosophy and goals of care, disease process and progression, self-care management, and end of life process and expectations.
Qualifications
Graduated of a state approved school of nursing. BSN preferred.
Current active, unencumbered license as a nurse in the State of Ohio.
Hospice and Palliative Nurse Certification (CHPN) preferred.
Previous experience in hospice/home care setting and with two years management or supervisory experience.
Proven ability to work within an interdisciplinary setting.
Must demonstrate acceptance of philosophy and goals of Hospice.
Current CPR certification (Basic Life Support).
Must possess a valid Ohio driver’s license, reliable automobile, and an acceptable driving record. Must have auto insurance and meet McGregor carrier underwriting standards
