PrimaryOne Health is currently seeking Lead STI Program Coordinator
Essential Job Responsibilities:
Program Coordination Functions:
Knowledge of Ryan White programming, HIV/AIDS risk behaviors, STIs, human sexuality, substance addiction, and HIV behavior change principles and strategies.
Coordinate the monitoring of grant deliverables and seek new grant opportunities in conjunction with P1H strategic plan
Participate in bi-weekly multi-disciplinary case conference meetings
Attends internal and community meetings as assigned and participates in Quality Improvement activities as assigned.
Participates in case management activities as they relate to population management for updating vaccinations.
Analyze federal, state and local epidemiological data and utilizes in day-to-day practices and planning.
Ensures and plans confidential and anonymous testing to the community in accordance with grant goals, as well as priority population and geographical needs.
Processes documentation required by state and local funding sources as required for public and private grants, including invoices and programmatic reports.
Monitors the recruitment, training and coordination of volunteers to assist with anonymous testing and HIV education.
Trains HIV/STI education to staff and volunteers as part of the orientation process and as needed.
Develops and implements goals and objectives relative to prevention, education and other care continuum programs.
Ensure that HIV population health services are implemented and delivered in a culturally and linguistically competent manner
Develops and fosters networking relationships with community and support resources.
Hires and supervises employees, assigns and schedules work, monitors work performance, handles personnel operations including: performance appraisals, promotions, transfers, dismissals, PTO and preparing timesheets.
Coordinate the scheduling of all HIV/STI Population Health staff, and clinic coverage for patient access.
Resolves problems and ensuring compliance with regulations and standards.
Continues professional training and education to advance knowledge and skills of HIV/AIDS.
Participates in continuous quality improvement of services rendered to CAN clients.
Utilizes resources and supplies in a cost-effective manner
Linkage to Care Functions:
Conduct individual sessions with individuals newly diagnosed with HIV.
Monitoring and coordination, through contact with client and community resource service providers, to ensure optimal utilization of offered services and adequate provision of needed services.
Establish and maintain up-to-date client records including demographics, proof of status, care plan implementation and evaluation of care.
Provide risk reduction education (i.e., safer sex education, and safer injection education) in order to prevent comorbidity risk and increased community viral load.
Assist in developing and maintaining a network referral system in the health care and social service community by becoming familiar with formal and informal services available to clients, reporting new services or changes in established services, and identifying gaps in available community services.
Connect clients to resources in order to access mental health, substance abuse, and other psychosocial needs.
Develop with clients a service delivery plan based on identified needs, available community resources, and financial status of client. Direct clients to needed services and coordinate and monitor access to and delivery of quality services.
Provide advocacy for clients as needed.
Conduct outreach and case finding activities as needed to keep clients connected to care
Input and maintain client level data in designated data system.
Conducts linkage to care, health education/literacy assessment, specialty care referrals and enrollment and eligibility verification activities on newly diagnosed persons and or out-of care persons.
Perform POC rapid HIV testing, pre/posttest counseling at fixed and community sites throughout the PrimaryOne Health System.
Develop and coordinate linkage agreements and service delivery with external community-based organizations.
Coordinates care with other health services team members as appropriate.
Documents all professional activities, contacts, and observations promptly and appropriately.
Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.
Travel to multiple health centers or community events required.
Other duties or special projects as assigned.
Successful candidates must possess a high school diploma or equivalent. Bachelor's degree preferred. Medical Assistant, LPN or other clinical background helpful. Experience understanding of HIV/AIDS/STI and a strong grasp of community resources (medical, mental health, and social service delivery systems) available to individuals living with HIV/AIDS. Point-of-care testing experience (or willingness to become certified/skilled in POCT).
