Description

Are you passionate about providing compassionate care to individuals in need? Do you have a special calling to support patients and families during their end-of-life journey? We seek a dedicated and skilled Hospice Aide to provide personal care services to patients. This position mainly covers southern Albemarle, Nelson, Buckingham, and Fluvanna counties.

This role involves providing personal care and/or homemaker services to patients under the guidance of a Registered Nurse. The goal is to support both the patients and their caregivers in a way that aligns with the Hospice philosophy, ensuring care is meaningful and compassionate.

In this role you will provide a wide range of responsibilities to ensure the well-being and comfort of patients. Key duties include:

Personal Care: Assisting with hygiene, toileting, mobility, medication, and meal preparation.

Health Monitoring: Measuring and recording vital signs, fluid intake/output, and reporting changes to the nurse supervisor.

Documentation: Maintaining electronic medical records (EMR) and documenting services provided.

Housekeeping: Performing light housekeeping duties in the patient area.

Patient Environment: Ensuring a clean, safe, and pleasant environment.

Infection Control: Following infection control principles.

Emergency Procedures: Recognizing and responding to emergencies.

Professional Development: Maintaining competency and registration.

Qualifications:

High School Diploma or GED

Registered by the Virginia Department of Health Professions as a Certified Nurse's Aide

One year relevant experience required

Hospice experience preferred

Must demonstrate basic computer skills.

Driver's license, personal automobile and current automobile liability insurance

Typical Physical Demands/Working Conditions:

Regular use of hands, reaching, stooping, kneeling, crouching, crawling, talking, and hearing. Frequent standing, sitting, climbing, and balancing. Occasional walking, tasting, and smelling.

Ability to lift/move up to 50 pounds and push up to 200 pounds in a wheelchair.

Effective telephone communication, keyboarding, and regular use of a computer screen.

Frequent travel
