PURPOSE
Provide case management to women and families in Emergency Housing.
Job Duties
1. Provide case management services and case coordination to Emergency Housing residents, and their children.
2. Conduct initial intake and exit interviews. Provide assistance with shelter information and referrals for women placed in emergency housing.
3. Assist women in developing/identifying/evaluating and revising a service plan, milestones and short and long-term goals.
4. Maintain and distribute supply of bus passes as needed
5. Assist residents in communicating concerns and needs to be addressed with resident assistants, for example: keys, supplies, etc.
6. Communicate and case conference regularly with program supervisor and team lead. Link residents to appropriate services and activities within and outside the YWCA, including employment and training services, legal assistance, substance abuse programs, housing, mental health and other appropriate services. Serve as liaison between YWCA and other social agencies and community resources.
7. Maintain accurate up-to-date electronic records, including documenting case notes within 24 hours of service. Monitor and follow up on residents' progress towards meeting goals.
8. Facilitate weekly Life Skills Workshops and resident meetings.
9. Demonstrates understanding of and sensitivity to cultural differences.
10. Demonstrates compliance with all appropriate HIPAA Privacy and Security Standards as defined by the YWCA of Rochester and Monroe County.
11. Perform other duties as requested.
RELEVANT BACKGROUND
Experience and Education Qualifications
Associates in social work, Psychology or Human Services with equivalent experience.
Two (2) years' experience in crisis intervention counseling.

When the grant or any subsequent renewal ends, grant employees will be terminated, and their positions will cease to exist.
