$5,000 SIGN-ON BONUS FOR FULL-TIME
This position is FT, 36 hours per week, every 4th weekend, and a holiday rotation.
Hours are either 9a-930pm, or 10-10:30 pm
Summary of Position: The Admissions Nurse is responsible for enrolling patients in the hospice programs and assisting with the referral and intake process.
Education and Experience:
Graduation from an approved School of Nursing.
Current RN license in the State of Ohio.
Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
Hospice experience preferred.
Responsibilities
Responsibilities of Position:
• Explain hospice services and benefits in regards to Medicare, Medicaid, and Private insurance, and know when to use Palliative Care Interventions and ABNs.
• Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
• Order appropriate Durable Medical Equipment (DME) for the patient.
• Perform competent initial nursing assessments.
• Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs.
In the absence of admissions and referrals, the admission nurse is responsible for the following:
• Care Center Assistance.
• End of Life/Continuous Care.
• Routine Homecare Visits.
• Chart Reviews.
• Other duties as assigned.
• Work evenings and weekends as needed to meet the needs of the patients and families.
• Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.

Qualifications
Special Skills and Requirements:
• Shall have a valid driver's license.
• Excellent customer service skills.
• Detail-oriented with excellent communication, documentation, and organizational skills.
• Ability to follow instructions.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to lift 25 lbs of supplies or equipment.
• Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking. Ability to assist with patient care activities such as ambulation, transfers, and positioning.
