Soliant Health is seeking a highly skilled and clinically driven Staff Pharmacist to join a reputable hospital in State College, Pennsylvania. This is a unique and rewarding opportunity for a pharmacist who wants to grow their clinical expertise by working in both inpatient and ambulatory care settingsa bination that is often unavailable at larger institutions where roles are more segmented.
As part of a collaborative pharmacy team, you'll have the opportunity to make a broad clinical impact across multiple areas of care. Whether you're working in the central pharmacy or rotating into the ambulatory care clinics, you'll be supported by a Director of Pharmacy who encourages clinical development.
Key Responsibilities:
Deliver prehensive central pharmacy services including order verification, dispensing, and providing accurate drug information.
Ensure safe, aseptic preparation of pounded sterile products, including high-risk medications, narcotic infusions, and cytotoxic chemotherapy.
Conduct ongoing drug therapy monitoring and clinical consults in accordance with protocols.
Rotate into ambulatory care roles, participating in direct patient management via collaborative practice agreements and therapy remendations.
Utilize Meditech as the health systems information technology platform.Requirements:
Bachelors or Doctorate in Pharmacy (BSPharm or PharmD)
Active Pennsylvania Pharmacist license or eligibility to obtain
pletion of a PGY1 residency or a minimum of three years of current clinical hospital experience
Strong proficiency in sterile pounding, clinical monitoring, and pharmacy operations
Experience with or readiness to engage in ambulatory care practicesSchedule: Variable day and afternoon shifts (primarily day shifts), 8-hour shifts
