Your Role: Embark on a fulfilling journey as our Hospice Aide / Certified Nursing Assistant (CNA)! Provide essential care and support in the sacred space of a patient's final journey, becoming a key member of our esteemed healthcare team.
Key Responsibilities:
Team Collaboration: Work closely with a registered nurse and other clinicians to deliver care according to the service plan.
Companionship and Care: Provide companionship and personalized care, encouraging independence and self-care in clients.
ADL Assistance: Assist clients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
Medication Support: Assist clients with the self-administration of medication as needed, ensuring safety and adherence to protocols.
Mobility Assistance: Provide mobility and transfer assistance within guidelines, promoting patient safety and comfort.
Meal Preparation: Prepare meals and snacks for clients, which may include grocery shopping according to dietary needs and preferences.
Light Housekeeping: Perform light housekeeping duties such as vacuuming, laundry, and linen changes to maintain a clean and comfortable environment.
Incontinence Care: Assist with incontinence care, ensuring client dignity and hygiene.
Transportation Support: Assist with transportation needs for appointments, errands, or outings.
Compliance and Documentation: Remain in compliance with HIPAA guidelines and all state and federal regulations. Document clinical notes clearly and concisely.
Equipment Management: Set up and maintain medical equipment and supplies, including Durable Medical Equipment (DME), ensuring readiness and functionality.
What We’re Looking For:
Valid CNA certification in the State of Pennsylvania
Experience in hospice or palliative care preferred.
Clear background check and drug screening.
Strong communication and empathy skills.
Self-starter, able to work independently and collaboratively.
Physical ability to perform job duties.
Reliable transportation and a valid driver’s license.
