We are looking for an MDS Coordinator to join our team. This person needs to be an RN or LPN.

Job Summary

The MDS coordinator is responsible for ensuring the completion of the MDS (minimum data set) assessment and transmission of the assessments to the Center for Medicare and Medicaid services. They utilize the data collected to develop a comprehensive care plan that addresses the emotional, physical and mental abilities of each patient. The plan is formulated in collaboration with the interdisciplinary team and the family and resident to provide goals for the resident's safety, long-term and short-term health. Other responsibilities include creating reports for the Center for Medicare and Medicaid Services using the minimum data set system.

Duties and Responsibilities:

Comply with the Facility's personnel, safety, and corporate policies and procedures

Comply with the nursing department's policies and procedures

Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager

Treat residents, family members, visitors, and team members with respect and dignity

Maintain the confidentiality of all resident and family information

Report to work on time and as scheduled

Conduct initial and periodic comprehensive, accurate and standardized assessments of each resident

Perform administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary

Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans

Ensure that a comprehensive care plan is completed within twenty-one days of the resident's admission

Participate in Facility surveys (inspections) made by authorized government agencies

Lead the Interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident; including measurable objectives and time tables to meet resident's medical, nursing, mental and psychological well-being

Participate in functions involving discharge plans as may be necessary

Maintain an effective, friendly working relationship with health professionals, physicians, consultants, and governmental agencies that may be involved in the resident assessment/care plan functions of the Facility

Conduct or coordinate the interviewing of each resident for the resident's assessment

Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents

Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care Job Requirements:

Must have a valid professional license (RN/LPN) that is in good standing with the State

Must have a valid CPR certification

If required to operate a motor vehicle for business purposes, must successfully complete the requirements contained in Company's Motor Vehicles policy

Must have basic computer skills

Must be able to read, write, speak, and understand the English language

Must be able to work beyond normal working hours and on weekends and holidays when necessary

Must be able to assist in the evacuation of residents during emergency situations

Must be able to perform the essential position functions of the job with, or without reasonable accommodation

Employee is required to use a transfer/gait belt or to obtain the assistance of another employee when attempting to assist residents when lifting or carrying objects in excess of 25 pounds. The employee must be aware that during the normal, routine performance of the essential functions, some of the following body movements may occur naturally, although hey may not be a requirement of the job
