Position Summary:

We are seeking a dedicated and compassionate Social Worker (MSW) to join the team. This full-time position will involve working 8-hour evening shifts and providing psychosocial assessments, crisis intervention, referrals, and information to patients and their families. The Social Worker will collaborate with the interdisciplinary team to achieve positive patient and family outcomes, ensuring that appropriate resources are in place to facilitate the discharge plan. Additionally, this role involves serving as a resource for less experienced social workers.

Required Qualifications:

Master's Degree in Social Work from an accredited university.

Coursework or continuing education in Evidence-Based Intervention.

Minimum of 1 year of social work experience in a healthcare setting (outpatient/acute care or skilled nursing facility). Preferred Qualifications:

Clinical Social Worker License (LCSW) upon hire.
