Licensed Vocational Nurse (LVN)

Job Description

Key Responsibilities:

Client Monitoring & Assessment:

Conduct regular vital signs checks (BP, pulse, respiratory rate, temperature).

Monitor clients for withdrawal symptoms, adverse reactions, and changes in physical or mental condition.

Perform nursing assessments and communicate concerns to the RN, physician, or clinical leadership.

Medication Observation:

Observe and document clients' self-administration of prescribed medications.

Ensure accurate Medication Observation Records (MOR) are maintained.

Monitor for proper medication compliance and immediately report any refusal, misuse, or diversion concerns.

Withdrawal & Detox Support:

Use withdrawal assessment tools (CIWA, COWS, etc.) to track symptoms.

Provide supportive care and interventions during detox process.

Communicate any significant changes or concerns to medical providers promptly.

Medical Documentation:

Maintain detailed, timely, and accurate nursing notes, assessments, and incident reports.

Ensure compliance with HIPAA, state regulations, and facility policies.

Client Care & Safety:

Educate clients on basic health topics and medication protocols.

Observe for any self-harm, safety, or behavioral issues and act accordingly.

Ensure a clean, safe, and supportive environment.

Team Collaboration:

Participate in interdisciplinary team meetings, shift reports, and case discussions.

Communicate effectively with counselors, therapists, physicians, and other staff.

Emergency Response:

Respond to medical and behavioral emergencies quickly and appropriately.

Administer basic first aid and CPR as needed; notify higher medical authority if advanced care is required.Company Description
