Job Summary:
Responsible for developing, implementing and monitoring tailored treatment plans in collaboration with clients, their families, and CST team members. Will provide direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention. Will coordinate and link clients to necessary community resources.
Requirements:
• Completion of a Bachelors Degree in a Human Service or related field with 2 years of post Bachelors experience with Mental Health or substance use challenges population, preferably in a community-based setting; meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health and a minimum of two (2) years documented experience with the population served…OR
• A non-human service Bachelors Degree may be considered if the candidate has 4 years of experience with the mental health population….OR
• An individual who holds a license of Registered Nurse (RN) in North Carolina, and also has 4 years of experience with MH/DD/SAS population….OR
• An individual who holds a Master’s degree in a Human Services field and has one year of full-time post-graduate degree accumulated MH/DD/SAS experience with the population served.
• Strong knowledge of evidence-based practices in mental health and substance use treatment.
• Excellent communication and interpersonal skills, with the ability to build rapport with diverse audiences.
• Strong organizational and time-management skills, with the ability to manage multiple projects and deadlines.
• Proficiency in Microsoft Office Suite and other relevant software.
• Ability to work independently or as part of a team.
• Flexibility to work outside of normal business hours if required.
• Knowledge of human services and NH/DD/SAS network
• Knowledge of State Services and Definitions as outlined in MH/DD/SAS
• Knowledge of basic office practices and procedures including record keeping and filing systems
• Knowledge of clinical diagnosis and treatments
• Valid NC Driver’s License, including personal vehicle insurance coverage and reliable transportation
• Current license or certification must be maintained, if applicable
Key Responsibilities:
• Conducts comprehensive assessments to identify client strengths, needs, and goals.
• Develops, implements, and monitors tailored treatment plans in collaboration with clients, their families, and CST team members.
• Participates in the development of the initial Person-Centered Plan (PCP) and its ongoing revisions and ensures its implementation
• Consults with identified medical and non-medical providers, engages community and natural supports, and includes their input in the PCP process
• Serves as support staff for the associate professional under the direction of the Team Lead.
• Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services
• Provides direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention.
• Coordinates and links clients to necessary community resources; assists individuals in meeting their vocational, housing, and other basic needs.
• Advocates for clients within various systems to ensure comprehensive support.
• Regularly monitors client progress and adjusts treatment plans to ensure effectiveness.
• Maintains detailed documentation in accordance with company, agency and regulatory standards; enters service notes and required documentation into the Electronic Health Record in a timely manner (within 24 hours). Any corrections will be entered within 24 hours of being notified.
• Ensures confidentiality regarding sensitive and protected information.
• Ensures individual rights to privacy and protected health information for the person supported.
• Assists Team Lead with service, agency, LME/MCO, state and/or federal documentation requirements and timelines such as NCTOPPs, PCPs/ITRs, and reminders relative to Clinical Monthly Summaries, Discharge Summaries, and Aggregate Reports.
• Participates in team meetings and case conferences to discuss and coordinate client care.
• Provides psychoeducation as indicated.
• Assists with crisis interventions and the development of relapse prevention and disease management strategies.
• Links clients to clinically appropriate services and arranges for necessary evaluations; assists clients with scheduling and linkage to community resources, under the direction of the Team Leader.
• Assists Team Lead with the coordination of discharge planning based on goals established at the initiation of service.
• Represents the company in a positive manner, reflective of the company’s mission, at all times.
• Engages in clinical training events and any other required meetings.
• Performs all other duties as reasonably required and assigned.
