Job Title: Social Worker (MSW)
Location: Fresno, CA 93703
Salary: $62,000-70,000/Annual - Based on experience
Job Type: Direct-Hire / Permanent role with full benefits!
Work Setting: Outpatient clinic specializing in geriatric care, with occasional home visits as needed.
Work Schedule: Day shift - 40 hours weekly (Mon to Fri)
Overview: The Social Worker will play a crucial role in delivering social services support, including assessments, counseling, case management, and referrals. Responsibilities:
Participate in the initial assessments, care planning, and periodic re-assessments (minimally every 6 months) of participants in alignment with industry and departmental standards.
Document participant changes accurately and promptly, by medical record standards.
Maintain open and effective communication with other team members regarding participant status and needs.
Oversee the coordination of 24-hour care delivery, providing individual and family counseling when necessary.
Lead group counseling sessions and activities to support participants' emotional and social well-being.
Refer participants and families to relevant community agencies, acting as a liaison and advocate to ensure participants' needs are met.
Contribute to program and policy development in social work and actively participate in quality improvement processes.
Conduct home visits to participants’ private residences.
Perform other duties and projects as assigned.
Essential Requirements:
Master’s degree in Social Work from an accredited school of social work.
Must have reliable transportation, a valid driver’s license, and the minimum required state auto insurance.
2 years of experience working in a hospital, nursing home, or community-based setting is preferred.
1 year of experience working with frail or elderly population is preferred. If not, training will be provided upon hire.
Experience with individual, family, and group counseling is preferred.
Ability to understand and address the psychosocial issues faced by frail and chronically ill individuals and their caregivers.
Basic computer proficiency and familiarity with electronic communication tools.
