An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. The role of the ACTT Nurse is to work as a team member responsible for conducting assessments, addressing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, providing a range of treatment, rehabilitation, and support services.
Job Duties Include:
Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
Provide treatment, rehabilitation, and support services with some interventions
Provide therapeutic services to consumer in line with best practice to promote recovery and stability
Conduct comprehensive assessment of psychiatric history, mental status, diagnosis, physical and dental health, use of drugs or alcohol, education and employment, social development, and functioning, activities of daily living, and family structure and relationships
Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and the individuals treatment team
Basic Requirements:
Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
Valid driver’s license, auto insurance coverage and reliable transportation.
Pass criminal background check and other regulatory checks.
Provide proof of your education.
Education and Experience Required:
A master’s degree in Counseling, Psychology, Social Work or closely related Human Services area and licensure listed below.
Exceptional written, oral, interpersonal, and organizational skills.
High level proficiency in Microsoft Office- MS Word, Excel, Access, and PowerPoint. Or an equivalent combination of education and experience.
