Community Growth Behavioral Health, Inc. provides Outpatient Behavioral Health Services to children, youth, adults, and families living in communities surrounding the Charleston, West Virginia area. We provide services for individuals and families suffering from chronic health conditions, mental illness, behavioral disorders, or substance disorders. Our Clinical Director, licensed professionals, and paraprofessionals work to create individualized treatment plans specific to individual needs.

Community Growth Behavioral Health, Inc. has the following opportunity immediately available in its Charleston, WV location:

Targeted Case Manager (Bachelor’s degree or higher in a Human Service field (required): $21 - $27 per hour DOE)

Experienced Targeted Case Managers will assess, refer, monitor, and link individuals to community services and supports. TCMs must have provided case management services for at least (1) years. TCMs must have a firm grasp of clinical processes and documentation.

Candidates must be team players and goal oriented with a natural drive to be successful and to be of service to others. Must possess sound ethical work standards and the ability to provide excellent quality care to individuals served. Contractor rates and salary positions are determined based on work experience, skills, special training, and abilities.
