We're currently looking for a Registered Nurse (RN) Case Manager to join our team and help our patients stay safe and healthy in the comfort of their own homes in Summit & Stark Counties.

What We're Looking For:

We're searching for a compassionate, reliable RN who:

• Has at least 1 year of RN experience (bonus points if it's in home health or geriatrics, but not required).

• Communicates clearly and kindly with patients, families, and the care team.

• Loves working with others and supporting both patients and coworkers.

• Is organized and can manage their day independently.
