Job Description
The Transition Case Manager works under the direction of the TIL Program. This position will successfully engage and retain young people in after care referred by LUK placement program(s). Through assessments and consultation, the Transition Case Manager will work with the young person to develop and implement after care plans and goals. Evaluation of progress toward goals in the aftercare plan are expected. This position will build connections between LUK and key partners and work with young people to achieve their goals while adhering to LUK’s core values.
To provide aftercare case management, referrals and advocacy as they relate to the well-being and development of the youth in accordance with LUK’s policies. Use available resources within a diverse community in a supportive, effective manner to establish the most appropriate service plan for the young person.
Qualifications: Bachelor’s Degree in Human Services or related field preferred. Position requires a commitment to young adult preservation and reunification as well as an understanding of the impact of childhood trauma and child development. Driver’s license and legally registered vehicle required.
