Job Title: Home Health Aide

Reports To: Home Health Director
Position Summary: Performs duties under the supervision of the Home Health Case Manager, Physical and Occupational Therapists. Performs home visits for the purpose of providing Home Health Aide services according to a written plan of care. Assists in the office under the supervision of the Home Health Director to help ensure compliance and billing readiness for the agency. Is responsible for managing and facilitating the JCH Toe Nail Clinic; Toe Nail - B/P Clinics in the surrounding area; Lifeline Medical Alert Program.
Schedule:
Working hours are Monday through Friday.
Duties & Responsibilities:
As part of the Home Health team the HHA enables patients to stay in their homes by monitoring and recording patient conditions, providing support, personal services and educating teaching patients and families as needed.
Supports patients by providing personal services such as bathing, dressing and grooming.
Supports patients by providing housekeeping and laundry services, shopping for food and other household requirements, preparing/serving meals and snacks and running errands.
Monitors patient condition by observing physical and mental condition, vitals, skin assessments, intake/output and exercise.
Helps family members or caregivers by teaching ways to appropriately lift, turn and reposition the patient.
Assists patients with nutrition choices, cleanliness and housekeeping.
Notifies Home Health Director, RN Case Manager, Physical or Occupational therapists of changes and patient needs, or of any changing or unusual conditions. Report includes physical and emotional condition or family situation in a timely manner to ensure appropriate action is taken as needed; ensures records are clear, concise and accurate.
Maintains a safe secure and healthy patient environment by following asepsis standards and procedures, maintaining security precautions and following prescribed dietary requirements and nutritional standards.
Protects the home health care agency by adhering to professional standards, home care policies and procedures, federal, state and local requirements.
Participates with the team for all Case Conferences or as requested for patient care conferences.
Performs any other duties deemed necessary by the Home Health Director.
Enhances service reputation by accepting ownership for accomplishing new and different requests, participates in problem solving for patients and the agency and exploring opportunities to add value to job accomplishments.
Qualifications & Skills:
Must be employed by the Johnson County Home Health (JCHH) a service provided to the area and surrounding communities by Johnson County Hospital (JCH).
Meets all requirements under JCH and JCHH policies for Home Health Aides.
Maintains a private vehicle and his insurance coverage for he/her own transportation for making home visits.
