Nurse Practitioner - Primary Care Clinic We are seeking a dedicated Nurse Practitioner for a primary care outpatient clinic in Columbus, OH.

Job Details Employment Type: Permanent Schedule: Monday - Friday, 8 AM - 5 PM Salary: $105,000 - $135,000/yr (Depending on Experience) Job Requirements Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC Active, non-probationary state Nurse Practitioner license Active DEA license US Work Authorization At least 1 year of experience in primary care: internal medicine, geriatrics, or family medicine preferred Responsibilities Provide exceptional primary care to patients Conduct office visits for routine and acute issues Administer annual wellness visits and health risk assessments Coordinate care with other healthcare providers, specialists, and agencies Lead population health initiatives in coordination with the Care Team Assist with phone triage and outreach Educate patients on health conditions, care plans, and treatments Participate in promotional healthcare activities Conduct home visits as needed Join our healthcare team today and make a difference in the lives of patients in underserved communities.
