Essential Functions and Responsibilities:
Conduct telephone triage for patient health complaints using an electronic triage assessment tool (Schmitt-Thompson)
Navigate multiple electronic platforms to assess current health complaint, patients’ electronic chart, PCP appointment scheduling, and ancillary center scheduling have appropriate and timely documentation
Document phone call details in tracking system for department data tracking and collection.
Assess, identify, and close patient care gaps.
Follow and execute all policies, procedures, and job responsibilities instructed by direct supervisor.
Develop positive and effective working relationship with COPC staff, offices, and physicians.
Qualifications:
Education, Licensures & Certifications
Active and unrestricted registered nurse license in the state of Ohio.
Two years minimum Registered Nurse experience in a hospital and/or medical practice setting
Knowledge, Skills & Abilities
Experience in telephonic nursing triage, preferred but not required.
Exceptional active listening, communication, and documentation skills, required.
Excellent customer service skills, and mind-set, required.
Ability to collaborate with a team, required.
Ability to adapt to department growth, expansion, and policies/ procedures, required.
Experience in pediatrics required.
Basic knowledge of Microsoft Office Programs, such as Excel, required.
Proficient knowledge of electronic health records, required.
