JOB TITLE: Direct Support Document Coordinator
DEPARTMENT: DSO
SUPERVISOR: DSO Manager
General Job Description:
Support our individuals and staff with all of their needs. Ensure their health & safety needs. Assist them with their vocational tasks, be a mentor and example to all. Train and mentor staff in the programs and ensure the smooth running of daily activities and maximum independence of our individuals’ needs in the program. Adhere to the budget of each program.
MAJOR DUTIES AND RESPONSIBILITIES:
Assist with completing any nursing/medication docs & attendance of monthly meetings
Write individuals’ IPs and complete vocational assessments & turn in 2 weeks prior to the meeting to the case manager
Ensure New Hire Orientation packets are followed, completed & turned into the Training Specialist in a timely fashion
Pass Meds as needed
Oversee daily attendance logs and enter data into Excel spreadsheet
Provide 1:1 documentation training to staff
Review all program books and ensure accuracy
File all monthly data appropriately
Review “monthly summary” reports & use the information to write individuals’ reports
Complete all end of month logs and enter data into Excel spreadsheet
File staff in-services appropriately once the staff have signed off on them
Ensure annual consent packets are returned in a reasonable amount of time
Ensure all assessments are up to date
Ensure that there is documentation to support the URR, PRC, HRC request forms
Ensure new monthly documentation is provided to staff and placed in program books by the 1st of every month (goals, daily logs, etc.)
Complete all Emergency Fact sheets
Ensure billing documents get to the billing office by the 2nd of the month
Perform Mega File Checklists Q6mons
Attend client meetings
Ensure measurable goals are created
Provide supervision to individuals during morning & afternoon transport transition times
Schedule all VOC activities
Have knowledge & follow all of the participant’s water safety, fall risks, behavior, toileting & LON requirements
Provide assistance to peers as needed
Assist in providing a positive program environment to staff and participants
Assist staff & individuals during fire drills and other emergency situations
Communicate with families, residences, & case management as needed
Be a team player
Learn & follow all agency P&P
Learn & follow all emergency & safety procedures for the agency
Be involved in OWC fundraising efforts
Perform other duties as they arise
Maintain mutual respect & professionalism for all co-workers & individual’s daily
Be a team player
Learn & follow all policy & procedures of the agency
Learn and follow all safety & emergency procedures for the agency
Be involved in OWC fundraising events
Perform other duties as assigned, or as they arise.
QUALIFICATIONS FOR THE JOB:
GED/High School diploma
3 years’ experience in a DSO setting
Must be reliable, dependable, flexible with your hours
Must possess proven experience and success
Must possess good organization, time management, and math skills
Must possess CPR/First Aid, Med Cert, PMT
Demonstrate adaptability, analytical and problem-solving skills
Ability to meet strict deadlines
Must possess valid driver’s license and have 5 years’ experience clean driving
Must be able to work as part of a team as well as be independent

Associates degree in related field or High School diploma + experience in Management
3 years’ experience in a DSO setting
Must have good document organization and math skills
Must be reliable, dependable, flexible with your hours
Must possess proven experience and success
CPR/First Aid, Med Cert a plus
PMT certification a Must
Must possess valid driver’s license and have 5 years’ experience clean driving
Must be able to work as part of a team as well as be independent
Must be willing and able to learn computer skills and Office 365
