We are seeking a Primary Care Advanced Practice Provider responsible for providing comprehensive primary care to patients in outpatient settings. The APP evaluates, diagnoses, treats, and manages acute and chronic medical conditions across the lifespan while emphasizing health promotion, disease prevention, and patient education. The Nurse Practitioner or Physician Assistant practices in collaboration with one or more physicians on his/her Standard Care Arrangement.

Qualifications:

Graduate of an accredited Physician Assistant or Family Nurse Practitioner or Adult Gerontology nurse practitioner program

Current national certification as Physician Assistant or Nurse Practitioner.

Current state license and registration.

Valid DEA license

Certified in CPR and ACLS endorsed by the American Heart Association.

Eligible for outpatient clinical privileges

Responsibilities:

Performs comprehensive health assessments including patient history, physical exams, and review of diagnostic studies.

Diagnoses and manages acute and chronic health problems in accordance with evidence-based guidelines and standards of care.

Develops, implements, and evaluates individualized treatment plans for patients in collaboration with or under the supervision of a physician.

Prescribes pharmacologic and non-pharmacologic therapies according to state licensure and institutional policy.

Orders and interprets laboratory, radiologic, and other diagnostic tests to inform clinical decision-making.

Educates patients and families on health promotion, disease prevention, and the management of chronic conditions.

Coordinates care with other members of the health care team and makes appropriate referrals to specialists or community resources as needed.

Monitors the effectiveness of interventions and communicates to the collaborating/supervising physician as needed.

Maintains accurate and timely documentation of all patient encounters and communications per MedOne and institutional guidelines.

Ensures continuity of care through appropriate follow-up and communication with patients and interdisciplinary team members.

Communicates treatment plan between patients, family, and the medical team.

Maintains patient and family confidentiality, privacy, and patient rights.

Participates in quality improvement initiatives and practice-based research as applicable.

Bills or logs encounters as directed.

Serves as a role model and resource to other advanced practice clinicians and clinical staff.
