Maintains accurate patient records, evaluates care effectiveness, and consults with physicians on care plan adjustments.
Mentors new staff, educates patients and families, and participates in quality improvement programs.
Completes mandatory training, complies with ethical standards, and maintains confidentiality.
Qualifications include an LPN/LVN license, at least one year of experience, a valid driver’s license (where applicable), CPR certification, and background checks.
Preferred candidates meet state-specific requirements, including screenings and certifications.
Works primarily in patients’ homes, with physical ability to lift up to 35 pounds, and uses common office equipment.
