Scheduler

SUMMARY: The Scheduler is responsible for coordinating and maintaining participant, staff, and service schedules to ensure seamless operations within the PACE organization. This role involves creating efficient schedules, managing changes, and communicating updates to all relevant parties. The Scheduler works closely with interdisciplinary teams (IDT) to ensure timely service delivery, optimize resources, and support high-quality care for participants. Accuracy, organization, and effective communication are key to success in this position.

ESSESNTIAL DUTIES AND RESPONSIBILITIES:

Develop and manage schedules for participants, staff, and services, ensuring efficiency and accuracy.

Ensure schedules are accurate, complete, and compliant with BoldAge PACE policies and participant care plans.

Coordinate participant appointments, transportation, and services across various care settings, including clinics, home care, and community locations.

Communicate schedule updates and changes promptly to participants, caregivers, and staff.

Collaborate with interdisciplinary team members to prioritize and address urgent scheduling needs.

Maintain and update scheduling systems, ensuring accurate documentation and compliance with organizational standards.

Monitor and adjust schedules to optimize resource utilization and minimize disruptions.

Address scheduling conflicts and resolve issues in a timely manner.

Ensure transportation and support services align with participant care plans and organizational requirements.

Track and report on scheduling metrics to identify areas for improvement and implement solutions.

Participate in quality improvement initiatives.

Provide administrative support related to scheduling, including data entry and filing as needed.

Uphold confidentiality and data security in all scheduling activities.

Assist with special projects or other duties as assigned to support operational goals.

EDUCATION AND EXPERIENCE:

High school diploma or equivalent (required).

1–2 years of experience in scheduling, administrative support, or healthcare operations (required).

Familiarity with scheduling software and electronic health records (EHR) systems (preferred).

Experience in a healthcare or PACE-related setting (preferred).

1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).

Proven ability to manage multiple tasks and coordinate complex schedules efficiently.

PRE-EMPLOYMENT REQUIREMENTS:

Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.

Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
